New Patient Forms*
We ask that you arrive about 10 minutes early to your first appointment to complete our new patient forms.
*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
Your First Visit
Your first appointment will be approximately one hour.
At the beginning of your appointment, your dentist will review your medical and dental history and complete a comprehensive dental exam. Your exam will include an oral cancer screening, dental x-rays, and a thorough examination of your gums, teeth, bite and jaw.
Based on your history and examination, your dentist will develop a long-term dental treatment plan for you. Your dentist will also take the time to answer any questions you have about your dental health and treatment options.
There is parking located beneath both Canada Place and the Citadel Theatre. There are also parking meters around both buildings and pay lots located near the office.
In order to provide a quality dental practice, appointments are required for all visits. Walk-in appointments are not accepted.
- Booking an Appointment
To book an appointment at Canada Place Dental, please contact our office during regular business hours.
Our staff will book a specific time for your dental visit to ensure that you receive the appropriate and comprehensive oral care that you need and deserve.
- Emergency Appointments
If you are experiencing a dental emergency, contact our office right away.
We try to help patients in need by providing same-day emergency care when possible as we know how painful a toothache can be. Our office is open five days a week for your convenience.
- Rescheduling or Cancelling an Appointment
If you are unable to keep a scheduled appointment, we ask that you call us with at least 24 hours notice, so that your appointment time may be given to another patient.
Otherwise a late cancellation/no show charge may be applied to you, which is not covered by your insurance.
- Arriving Late to an Appointment
Punctuality is a priority in our practice. We understand that unexpected issues may arise that may cause a late arrival.
If you do arrive late, we will attempt to accommodate you without disrupting our busy schedule.
Unfortunately, if you arrive more than 10 to 15 minutes late, we may be unable to provide you with your planned treatment and you may need to reschedule your appointment.
Insurance & Direct Billing
At Canada Place Dental, we accept most insurance plans. As a courtesy to you, our office will submit your claims to your insurance carrier. We offer electronic submission (EDI) where possible to expedite the claim process. When more extensive treatment is planned, we will send a detailed estimate or predetermination to the insurance carrier(s) involved.
For your convenience, we accept cash, personal cheques, money orders and major credit cards including, VISA, MasterCard and AMERICAN EXPRESS. Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.
